Use this service if you have been told to stay at home because of coronavirus and you need a note for your employer.
This service is only for people who:
- have symptoms of coronavirus and have used the 111 online coronavirus service

If you are not sure if you need to stay at home, get the latest NHS advice on coronavirus.
If you have to stay at home but feel well enough to work, ask your employer if you can work from home. If you can work from home, you will not need an isolation note.
You can also use this service for someone else.
For Sick notes NOT related to Covid-19/Coronavirus: Please note that you do not require a certificate if you are off work for one to three days.
If you are off for four to seven days inclusive, you should complete a self certificate form (SC2) which can be obtained from your employer or the DSS; if you are self employed you will need to use form SC1, which is available at reception.
If you are off for eight or more consecutive days your doctor will complete a Social Security form Med 3 or Med 5. Medical certificates for illnesses less than eight days are not required by law. A private certificate can be supplied, but a charge will be made for this service.
