If you wish to register as a patient you will need to complete some forms, which are available to download from this website (see below). Once you have completed the forms you can either scan them and email them to us at [email protected] or post them to the surgery.
Medical treatment is available from the date of registration. You do not need the consent of your previous doctor.
You will need to send in a form of identification with you when you register. Any of the following are acceptable forms of ID:
We do not need to see the originals; we are happy to receive scanned copies via email or photocopies through the post.
You will also need proof of previous address and proof of current address such as a utility bill or tenancy agreement.

You can download and print off the forms you need to register with us from here. Every applicant needs to complete two forms:
- NHS registration form for General Medical Services (GMS)
- A new patient health questionnaire:
Change of address
Please inform a receptionist by telephone as soon as possible so that our records can be updated. This may be of particular importance if you require a home visit in an emergency. We are pleased to register anyone who lives within the practice area.
